When I was younger, I'd use Campaign Cartographer and Fractal Mapper. Fractal Mapper is really good at making randomly generated fractal coastlines that follow the general path that you drew. The terrains aren't as realistic as Campaign Cartographer's but it's cheaper.

Now, I use 2D AutoCAD and Photoshop. This combination gives me a better level of sophistication when doing maps. You can see an comparison below of how I used AutoCAD and PhotoShop to clean up one of the old maps for Caverns of Thracia.

I use Fractal Mapper too, Mostly for Traveller stuff.

For fantasy maps, I use the commercial Paint Shop Pro. These days it's called Corel Painter and is available here:

I also use GIMP which is a free art application with plenty of good plugins. You can even use some Photoshop brushes and plug-ins as well

Gimp is available here:

My other big art and mapping application, is also free, it is called Paint.Net, and it is pretty awesome as well.

Paint.Net free download:

I recommend Inkscape. I use CorelDRAW personally but Inkscape has all the Corel features I use.

I also keep up with Profantasy's Campaign Cartographer. Mainly for their useful symbols, fills, and other extras.
